Search "lab notebook required entries" and most results only tell you what belongs on the cover page — project number, principal investigator, period of use. We already covered those in how to fill out the cover page. What's missing is what comes after the cover page: every time you write up one experiment, one page, there's a separate set of items that has to be there, and skipping them gets flagged at review even when the cover page is perfect. Here's what Korean university research-office manuals and the national research-note guideline agree belongs in the body of every entry.
What every single entry needs
Pulling together the research note manual published by Seoul National University's research office and KAIST's technology-transfer center summary, a single logged experiment needs the following together in one place.

The common thread is the opposite of "just write down the result." A page that lists results with no purpose or method attached leaves no way to later prove "this result actually came from this procedure at this time." Purpose, method, results, and interpretation together, on the other hand, tell a coherent story: that this person actually carried out this process at this point in time.
The three things most often left out
- Dropping failed data. The most common shortcut — treating an
unsuccessful run as "not worth recording." In a patent dispute or a research-misconduct investigation, failed results are often what proves the work was actually done. A notebook with nothing but unbroken success reads, ironically, as more suspicious, not less.
- Signing in batches instead of daily. Rather than signing the page the
day it's written, entries pile up and get signed off all at once later. As covered in our post on signature requirements, a signature is meant to certify "this page was written at this time" — batching it after the fact defeats the point.
- Attaching data with no interpretation. Pasting in an instrument
printout or photo and stopping there. An attachment only counts as part of the notebook if the body text says, even in one line, what it is and why it's there.
The cover page and the body entries do different jobs
The cover page proves "whose notebook is this, for which project, covering what period." The body entries prove "what was done on this specific page, when, and how." Filling in only one leaves you with half a proof — a perfect cover page with no purpose or method in the body reads as "correctly formatted but substantively thin," while a thorough body with an empty cover page reads as "loose pages with no traceable source."
Electronic notebooks fill in some of this automatically
A paper notebook needs the date and signature written by hand. An electronic lab notebook instead records a save-time timestamp and the author's account information in the system automatically. That doesn't mean the date and signature fields stop mattering — it shifts the question to whether the system is recording that information in a way that can actually be trusted. Checklist before you write an entry

- [ ] Are the title and purpose written before the results?
- [ ] Is the method/conditions detailed enough that someone else could
reproduce it?
- [ ] Are unexpected results and failed data kept as-is?
- [ ] Is there at least one line of interpretation after the results?
- [ ] Was the page signed the same day it was written, not in a batch
later?
- [ ] Does every attached photo or printout have a note explaining what it
is?
